Account Switch Kit Forms

Account Closing Request

This letter will notify your old bank of the accounts you are closing and where remaining funds should be sent.

Direct Deposit Change Request

Use this form to notify your employer or other source of direct deposit funds to begin deposits into your new Mid American account.

Automatic Payment Transfer Request

Send to each of your current automatic payment vendors (utilities, mortgage, insurance, etc.) to have future payments deducted from your new Mid American account.

Automatic Payment Authorization Request

Send to new vendors to sign up for automatic payment from your Mid American account.

Automatic Payment Transfer Cancellation Request

Use this form if you wish to cancel an automatic payment being deducted from your old account.
