This letter will notify your old bank of the accounts you are closing and where remaining funds should be sent. |

Use this form to notify your employer or other source of direct deposit funds to begin deposits into your new Mid American account. |

Send to each of your current automatic payment vendors (utilities, mortgage, insurance, etc.) to have future payments deducted from your new Mid American account. |

Send to new vendors to sign up for automatic payment from your Mid American account. |
|
|
Use this form if you wish to cancel an automatic payment being deducted from your old account. |
