Printable Desktop Banking & Transfer Authorization Form
Please fill in the appropriate sections, print the form, sign it and mail it to Mid American at the address on the bottom or fax it to us at (316) 722-0920. (Click Here for print friendly version)

In order to use Home Banking, you will need to specify both a PIN (Personal Identification Number) and a password in writing, using the Desktop Banking and Transfer Authorization form. Your Desktop Banking PIN and your MATT (Mid American Telephone Teller) PIN will be the same. If you know your MATT PIN, please enter it on the form. If you do not know your MATT PIN, please select one for Desktop Banking and MATT and enter it on the form. Desktop Banking/ MATT PINs are 4 digits long. Passwords should be at least 6 characters, and up to 10 characters long. If you wish to transfer funds from your account to one or more other member accounts, you will need to fill out the Transfer Authorization section.
